We are currently enrolling. Our books are open to all new patients.
To enrol with New Brighton Health Care, please complete the online enrolment application via the link below. You can also attach a photo of your Driving license, passport or other id, as well as any work visa information (this form works best in Chrome)
Alternatively, you can download and print a physical form and bring that into the medical centre or post it to New Brighton Health Care, 22 Union Street, Christchurch, 8061.
Your first consult will need to be paid for in full prior to attending.
- Please note that every patient over the age of 16 must complete or sign their own enrolment form.

What is HealthOne?
HealthOne is a South Island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information. HealthOne adheres to the principles of the Privacy Act 2020 as well as the Rules set out in the Health Information Privacy Code 2020. Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested.
